Wellness Center Membership Requirements

Membership is required to use the Wellness Center. Access is granted upon completion and submission of the appropriate registration forms listed below.

• Postdoctoral Fellows
• Residents and Fellows

Residents and Fellows - Additional Requirements

Residents and Fellow must complete the Residents and Fellows Waiver and Release form.

Form Submission Instructions

• Medical Residents and Fellows: Return completed forms to the GME Office (LM068).
• Dental Residents and Fellows: Return completed forms to the Wellness Cetner (LM034).
