Starting Tuesday, Sept. 1, UConn Health will launch the new Capacity Management Center, leveraging the Epic Transfer Center module to streamline and optimize patient transfer workflows between UConn Health and community partners hospitals.

With anticipated growth in referral volume and expanding partnerships, the current manual, paper-based process is no longer sustainable. The existing workflow can result in incomplete or missing information during transitions of care, creating additional challenges for care coordination and patient movement.

The new Transfer Center will support safer, more efficient patient transfers by enabling real-time data entry directly into Epic. This will help reduce documentation errors, eliminate paper forms, and improve the availability and accuracy of information throughout the transfer process.

In addition, the solution will enhance coordination of patient movement across the UConn Health system, supporting greater operational efficiency, improved communication, and better clinical outcomes.

The launch of the Capacity Management Center represents an important step toward modernizing our transfer processes and strengthening coordination across UConn Health and our hospital partners.
